Please bring your child's birth/discharge summary, vaccination card, and any prior prescriptions or lab reports. If your baby spent time in a NICU, bring that discharge summary too.

Every child is given a full EPI-based vaccination schedule, and the clinic sends reminders ahead of each due date so you never miss a dose.
